Breast augmentation is by far the most requested cosmetic surgery among women. In Italy, according to AICPE statistics, over 33,000 breast augmentation surgeries were performed in 2013, making it the most popular procedure for women. According to ISAPS (International Society of Aesthetic Plastic Surgery), more than 3 million cosmetic surgery procedures were carried out worldwide in 2016.

The shape and features of the implant play a crucial role in determining the final appearance of the breast, and choosing them is not simple. In 2017, the most requested were round micro- or nano-textured implants with ergonomic gel that adapts to the body’s position.

Breast augmentation, by altering the proportional balance between breasts and chest and between breasts and body as a whole, does not allow for a single universal standard in implant choice. The main considerations for the plastic surgeon during the preliminary consultation are the type of reshaping desired, the patient’s tissue type, and her anatomical features.

For example, anatomical implants greatly enhance the profile’s projection but are not suitable for creating a fuller décolleté in women with very small breasts. The shape and volume of the new breast are determined by two factors: the chosen implant and the volume and position of the original breast tissue.

The choice of implant volume is always preceded by a tissue analysis of the area. The implant should fill the skin envelope without being excessive. Very large implants must be carefully evaluated, as oversized implants are associated with a higher risk of complications and, in the long term, may lead to dissatisfaction.

For the choice of implant shape, the plastic surgeon assesses the patient’s anatomy. The width of the breast implant should not exceed the base dimension of the gland, while implant height is evaluated according to the proportions of the chest and shoulders. Even the shape and projection of the buttocks may be considered to create aesthetic continuity with the patient’s natural body proportions.
